TN School Immunization Requirements (2017-18)
A Tennessee Department of Health Certificate of Immunizations is required for all Kindergarteners, 7th graders and new students registering for school.
Please Note:
- Schedule your child's checkup now and avoid the rush.
- Kindergarten immunizations can be given any time after your child turns 4 years old.
- Please ask the nurse at your child's next checkup for a copy of the TN Immunization form.
- If you call or come by the office to request a copy of your child's TN Immunization form, please allow us 72 hours to complete the request.
