TN School Immunization Requirements (2017-18)

Tennessee Department of Health Certificate of Immunizations is required for all Kindergarteners, 7th graders and new students registering for school.

Please Note:

  1. Schedule your child's checkup now and avoid the rush.
  2. Kindergarten immunizations can be given any time after your child turns 4 years old.
  3. Please ask the nurse at your child's next checkup for a copy of the TN Immunization form.
  4. If you call or come by the office to request a copy of your child's TN Immunization form, please allow us 72 hours to complete the request.
Children enrolling in Kindergarten Children entering 7th Grade Children newly enrolled to TN schools(not in K or 7th)
DTaP (DT if appropriate): 5 doses Tdap booster: not required if a Td booster dose given less than 5 years before 7th grade entry DTaP (DTaP, or DT if appropriate): 5 doses
Hepatitis B (HBV): 3 doses Verification of immunity to varicella: 2 doses of vaccine or history of disease Hepatitis B (HBV): 3 doses
Measles, Mumps, Rubella: 2 doses of each, usually given together as MMR   Measles, Mumps, Rubella:2 doses of each, usually given together as MMR
Poliomyelitis (IPV/OPV): final dose on or after the 4th birthday   Poliomyelitis (IPV/OPV): final dose on or after the 4th birthday
Varicella (Chickenpox vaccine): 2 doses or history of disease   Varicella (Chickenpox): 2 doses of vaccine or history of disease
Hepatitis A: 2 doses, spaced at least 6 months apart
